Medical examination report for a lorry or bus driving licence (D4)

A doctor will need to complete this form if you are applying for a lorry or bus driving licence.

Some doctors will be able to fill in both the vision and medical assessment sections of the report. If your doctor is unable to fully answer all the questions on the vision assessment, you must have it filled in by an optician or optometrist.

D2 application form

D2 application forms are available from your nearest Post Office® that deals with driving licence applications. Find your nearest applicable Post Office at https://www.postoffice.co.uk/branch-finder

As of 20 June 2019, DVLA stopped accepting medical applications (D4) completed by Doctors on Wheels Ltd in June 2019.
